For referring doctors
How to refer a patient
Send the referral form with the reason for the referral and any tests you have already done. The center takes it from there.
- 01
Step 1: You send the form
The form takes the patient's name and phone, your practice, and a line on why you are referring.
- 02
Step 2: We call the patient
Our referral coordinator calls the patient to book and checks insurance before the visit.
- 03
Step 3: The evaluation
One of our optometrists runs the dry eye tests and goes through the results with the patient.
- 04
Step 4: A summary comes back to you
We send a written summary: the tests we ran, what they showed and the plan we agreed with the patient.
What we do, and what we send on
We handle
Dry eye evaluations, eyelid gland imaging, home care plans, prescription drops and in-office treatments the doctors are licensed to perform.
We co-manage
Patients whose dry eye comes with another condition, with a shared plan and shared notes. We keep you informed at each visit.
We refer on
Anything outside an optometrist's care. We tell the patient, tell you and send the records.
Questions from referring offices
What does the patient need to bring?
Their insurance card, a list of the drops and medicines they use, and their glasses and lens case.
Do you see patients without a referral?
Yes. A referral is welcome and never required.
Will you send the patient back to us?
Yes, with the summary. If the patient needs care outside our scope, we refer on and tell you.
Who do I call with a question?
The referral coordinator, at the office phone number.
